Abstract

The accuracy in combining loose sheets with other medical record files to be filed into the proper medical record folders greatly helps the quality of health services provided to patients. In controlling loose sheets, including supporting reports which include Service Guarantee (SJP), billing, laboratory results, prescription sheets and medical resumes that often come later to be filed into the appropriate folder. In the case of loose sheet storage activities, medical records officers at Bengkulu Rafflesia Hospital did not use the out guide to mark loose sheets when they were not at the storage location. This will have an impact on inaccuracy in the provision of diagnosis codes, the validity of billing and calculation of treatment costs as well as delays in submitting BPJS claims. The method in this community service activity is science and technology diffusion which is used in order to design an out guide to assist officers in arranging loose sheets on medical record filing racks at the Medical Record Installation of Rafflesia Hospital, Bengkulu. Furthermore, training was also carried out which began with extension activities and continued with training. Community service activities run smoothly. Initially loose sheets were stored in special boxes or hung on the sides of medical record storage racks without using folders or protective covers. After counseling and training, the officer finally knew that the out guide was a record of loose sheet use of medical records and at the same time a loose sheet location marker that was coming out of the medical record map, so that the exit and entry of loose sheets could be tracked clearly, quickly and precisely.
