- Increasing global mobility has led to greater cultural and religious diversity in healthcare settings, including in Japan, a country historically characterized by cultural homogeneity. One emerging challenge is the provision of culturally and spiritually sensitive midwifery care for Muslim patients. Limited understanding of Islamic values among healthcare providers may result in communication barriers, patient discomfort, and reduced quality of maternal care. This international community service program aimed to enhance the cultural competence and knowledge of healthcare students and professionals in delivering culturally and spiritually sensitive midwifery services for Muslim patients. The program was conducted in collaboration with the School of Biomedical Sciences, Nagasaki University, Japan, involving healthcare students and maternal health professionals. An educative–participatory approach was employed, incorporating interactive lectures, case-based discussions, and cross-cultural reflection sessions. Program effectiveness was evaluated using pre-test and post-test assessments to measure changes in participants’ knowledge levels. The results demonstrated a substantial improvement in participants’ knowledge, with those categorized as having very good knowledge increasing from 6.7% before the intervention to 80.0% after the program. Notably, no participants remained in the moderate or low knowledge categories following the intervention. These findings indicate that a contextual and interactive educational approach is effective in strengthening understanding of cultural and spiritual sensitivity in midwifery care. This community service activity is expected to contribute to improved quality of inclusive, ethical, and patient-centered maternal care in multicultural healthcare environments, while also fostering sustainable international academic collaboration in the field of maternal and reproductive health.
